demonstration projects addressing particular challenges to existing
models of hospice and palliative care.
Particularly targeted are:

including children, American Indian tribes, the seriously mentally
ill, veterans, isolated Alaskan villages, rural communities, and
urban African American patients.
- Specific
Diseases and Terminal Conditions
including end-stage renal disease, cancers, Alzheimer's and related
dementias, and advanced HIV disease complicated by addiction or
mental illness.
- Challenging
Clinical Settings
including nursing homes, cancer centers, dialysis centers, large
managed care organizations, and maximum-security prisons.
- Other
